Bone metabolism disorders in liver and gastrointestinal diseasesReview articles
Normal functioning of the digestive tract and the liver is important for normal bone metabolism. Patients with chronic cholestasis in primary biliary cirrhosis or primary sclerosing cholangitis and those with cirrhosis are most commonly affected by metabolic bone disease. Among gastrointestinal diseases, disorders of bone metabolism are found in celiac disease, ulcerative colitis, Crohn's disease, chronic pancreatitis, and in conditions following gastrectomy. Knowledge and anticipation of disorders of bone metabolism allow ear ly initiation of preventive measures and early diagnosis of severe bone metabolism disorders. Patients who are candidates for liver transplantation are the most vulnerable group. Osteoporotic fractures occurring after liver transplantation are so common that they need to be actively diagnosed and treated. Recommendations for the screening of bone metabolism disorders in at-risk patients with liver and digestive diseases are currently available. First, it is necessary to treat the underlying disease of the gastrointestinal tract and liver, and to supplement calcium and vitamin D. These measures often lead to improvements in bone mineral density. In severe cases, specific therapy for the treatment of osteoporosis is necessary.
