The publication is a joint position of the Czech Society of Clinical Biochemistry and the Society for Metabolic Skeletal Diseases on the use of bone turnover markers in patients with osteoporosis. It is recommended to use PINP (Procollagen type I N-terminal propeptide) in μg/L as a marker of bone formation and CTX-I (C-terminal telopeptide of type I collagen) in ng/L as a marker of bone resorption. Blood sampling should be performed after an over­night fasting, an enormous physical load 24 hours prior to collection should be also avoided. Always take blood samples between hours 7 : 00 and 10 : 00 in the morning. Both serum and EDTA-plasma are acceptable....

Mastocytóza je raritné ochorenie, ktoré je charakterizované abnormálnou proliferáciou mastocytov, so širokým spektrom klinických prejavov v dôsledku postihnutia rôznych tkanív a orgánov. Častou manifestáciou systémovej mastocytózy, najmä v dospelosti, je osteoporóza. Najčastejším miestom postihnutia je lumbálna chrbtica, v dôsledku väčšej straty trabekulárnej kosti v porovnaní s kortikálnou kosťou. Patomechanizmus vzniku osteoporózy pri mastocytóze je multifaktoriálny. K osteoporóze dochádza jednak v dôsledku neoplastickej infiltrácie kostnej drene mastocytmi, najmä však vplyvom uvoľnených mediátorov z mastocytu ako napr. histamínu, heparínu, prosta­glandínov,...

Treatment of atypical femoral fracture with teriparatide: case report

Atypická fraktura femoru (AFF) je vzácným nežádoucím účinkem dlouhodobé aplikace bisfosfonátů nebo denosumabu. Předkládáme popis případu pacientky léčené pro osteoporózu bisfosfonáty (celkem 104 měsíců). Na základě prodromálních příznaků byla zobrazovacími metodami (RTG, MRI, kostní scintigrafie) diagnostikována inkompletní atypická fraktura levé kosti stehenní. Přerušení podávání bisfosfonátu, odlehčení končetiny a šestiměsíční aplikace teriparatidu vedly ke zhojení zlomeniny. U pacientů užívajících antiresorpční léčbu osteoporózy po dobu delší než 4 roky je nezbytné pravidelné dotazování na prodromální příznaky AFF.

Primary hyperparatyroidism (PHPT) is a common endocrine disease and represents the most common cause of hypercalcemia. Thanks to modern diagnostic and laboratory approaches that allow us to diagnose and treat asympto­matic and mild forms of PHPT, the number of patients with clinically symptomatic PHPT decreases. The case report deals with the case of a 30 year old woman who underwent medical examination due to renal colic. Nephrolithiasis and multiple osteolytic lesions in the pelvic area were found on CT.
