Denosumab v léčbě osteoporózy - vysadit či pokračovatSaid at the congress

Na 68. sjezdu českých a slovenských revmatologů, který se konal 16.–18. října 2025 v Brně, zazněla přednáška, ve které MUDr. Sokalska-Jurkiewicz hledala odpověď na otázku, zda a po jaké době vysadit denosumab (Prolia®) v léčbě osteoporózy. Připomněla historii léčby osteoporózy a prokázanou účinnost denosumabu, který vede po celou dobu léčby k nárůstu kostní minerální denzity (BMD) a snížení rizika vertebrálních i non-verbálních zlomenin. Frekvence nežádoucích účinků je nízká a nestoupá ani při užívání přesahující 10 let. Řada českých pacientů užívá denosumab, který byl uveden na trh v roce 2010, již mnoho let a lékaři se často pozastavují nad otázkou ukončení této terapie. Dosud totiž neexistuje jednotný konsenzus o vhodné délce této léčby. Jak ale ukazují publikované práce, vysazení denosumabu může vést ke zvýšení rizika zlomenin obratlů, a nutná je navazující terapie vhodným přípravkem. Sekvenční strategie léčby mají ovšem svá omezení, a proto se jako nejvhodnější jeví pokračování v terapii denosumabem.
